The Kollel replies:
The Chasam Sofer in Kesuvos (41a, DH "ha'Omer") explains that it cannot be b'Makom Nezek. The pain is the Pisuk Raglayim and the Nezek is in the Sheviras Besulim. Since the Nezek is not b'Makom ha'Tzar, it cannot be evaluated as Tzar b'Makom Nezek.
