Ramban (18:29): There are three levels of Kareis: 1. A Tzadik (with majority Mitzvos) who eats Cheilev or Dam will die before the age of sixty, but his Soul will go to Olam ha'Ba; 1 2. If, in addition to eating Cheilev or Dam, he has more Aveiros than Mitzvos, then, he may well live to a ripe old age, but his Soul will be banished from the World of Neshamos; 2 3. Kareis that one receives for Avodah-Zarah and Megadef (someone who curses Hashem, Kevayachol), 3 in which case, one loses one's portion in both worlds. 4 And it is only where the Torah writes Ariri that a Chayav Kareis will die childless. 5

Rashi writes that Kares entails that his children and his days will be cut-off. In Semachos (3:10) it asks how this applies to an old person. It should ask also about one who has no children!
