(1) That is, the Mohel had another baby whose Milah had to be performed on Shabbos. Since it was permissible to desecrate that Shabbos for the sake of the second baby, if the Mohel erred in this Mitzvah and circumcised the wrong baby on Shabbos, but post facto fulfilled the Mitzvah of Milah for the baby that he circumcised, the Mohel is Patur (according to the Tana'im that exempt someone in such a case). If there was no Milah to perform on that Shabbos, the Mohel would be Chayav for accidentally performing a Milah on Shabbos, since he was not actively involved in a true Mitzvah.
(2) Since he performed a Mitzvah.
(3) Since he was worried and preoccupied with the performance of a Mitzvah, even though he did not perform a Mitzvah when he desecrated the Shabbos, he is Patur.
