(1) For example, the flesh was destroyed or lost, or it was taken out of the Beis ha'Mikdash, or it became Tamei.
(2) If the blood became Pasul before the Zerikah, it is obvious that the hide remains Asur according to everyone. This is written explicitly in the Tosefta.
(3) This was not common, since it was not the common practice to delay the Zerikah until after the skinning of the hide. Rather, the Zerikah was performed immediately after the Shechitah and Kabalas ha'Dam, before the hide was removed from the animal (Gemara).
