(1) For example, her husband divorced her (at which time she became prohibited to his brothers) and then remarried her (Mishnah, 109a).
(2) For example, at the time her husband died and she fell to Yibum, her sister was married to the Yavam. Later, her sister died (Mishnah 30a, 32a).
(3) For example, the Yavam married the sister of the Yevamah (Achos Zekukaso), and then that sister died. Alternatively, after the woman fell to Yibum, her sister (who was married to another brother) fell to Yibum and prohibited her to the brothers; then that sister died (our Mishnah; the Beraisa of "three brothers" in our Gemara).
(4) See Insights, for a discussion of the extent of the Heter.
