Malbim: Re'a is one who loves his friend at all times.
What is the meaning of "v'Ach l'Tzarah Yivaled"?
Rashi: At a time of affliction, the friend will be born to you like a brother to help you and join in your affliction.
R. Yonah: After the Kesil learns Chachmah, this begets affliction. He can harm people more than initially. (a) Before he learned, his words were not heeded, to make people stumble and lead them in a bad path. (b) After he learned, he will be shrewd to seek claims to aid Sheker and make Mishpat crooked. He will condemn a Tzadik and vindicate a Rasha. He and affliction of the time will be brothers, i.e. he will be added to affliction of the time.
Malbim: At a time of affliction, a Re'a increases love to the point that he will risk his life for his friend, as if he were his brother.
