שמעו נא רבותי דינים מומחים, כל נדר או שבועה או אִסָּר או קונם או חרם, שנדרתי או נשבעתי בהקיץ או בחלום, או נשבעתי בשמות הקדושים שאינם נמחקים, ובשם הוי"ה ברוך הוא, וכל מיני נזירות שקבלתי עלי, ואפילו נזירות שמשון, וכל שום איסור ואפילו איסור הנאה שאסרתי עלי או על אחרים, בכל לשון של איסור, בין בלשון איסור או חרם או קונם, וכל שום קבלה אפילו של מצוה שקבלתי עלי, בין בלשון נדר בין בלשון נדבה, בין בלשון שבועה, בין בלשון נזירות, בין בכל לשון, וגם הנעשה בתקיעת כף, בין כל נדר ובין כל נדבה ובין שום מנהג של מצוה שנהגתי את עצמי, וכל מוצא שפתי שיצא מפי, או שנדרתי וגמרתי בלבי לעשות שום מצוה מהמצות, או איזה הנהגה טובה או איזה דבר טוב שנהגתי שלש פעמים, ולא התניתי שיהא בלי נדר, הן דבר שעשיתי על עצמי הן על אחרים, הן אותן הידועים לי הן אותן שכבר שכחתי, בכלהון אתחרטנא בהון מעקרא ושואל ומבקש אני ממעלתכם התרה עליהם, כי יראתי פן אכשל ונלכדתי חס ושלום בעון נדרים ושבועות ונזירות וחרמות ואיסורין וקונמות והסכמות. ואין אני תוהה חס ושלום על קיום המעשים הטובים ההם שעשיתי, רק אני מתחרט על קבלת הענינים בלשון נדר או שבועה או נזירות או איסור או חרם או קונם או הסכמה וקבלה בלב, ומתחרט אני על זה שלא אמרתי, הנני עושה דבר הזה בלי נדר ושבועה ונזירות וחרם ואיסור וקונם וקבלה בלב. לכן אני שואל התרה בכלהון, ואני מתחרט על כל הנזכר, בין אם היו המעשים מהדברים הנוגעים בממון, בין מהדברים הנוגעים בגוף, בין מהדברים הנוגעים אל הנשמה, בכלהון אני מתחרט על לשון נדר ושבועה ונזירות ואיסור וחרם וקונם וקבלה בלב. והנה, מצד הדין המתחרט ומבקש התרה, צריך לפרוט הנדר, אך דעו נא רבותי כי אי אפשר לפורטם כי רבים הם, ואין אני מבקש התרה על אותם הנדרים שאין להתיר אותם, על כן יהיו נא בעיניכם כאילו הייתי פורטם.

הכל יהיו מותרים לך, הכל מחולים לך, הכל שרויים לך. אין כאן לא נדר ולא שבועה ולא נזירות ולא חרם ולא איסור ולא קונם ולא נידוי ולא שמתא ולא ארור. אבל יש כאן מחילה וסליחה וכפרה. וכשם שמתירים בבית דין של מטה, כך יהיו מתרים בבית דין של מעלה.

הרי אני מוסר מודעה לפניכם, ואני מבטל מכאן ולהבא, כל הנדרים וכל שבועות ונזירות ואיסורין וקונמות וחרמות והסכמות וקבלה בלב שאקבל עלי בעצמי, הן בהקיץ הן בחלום, חוץ מנדרי תענית בשעת מנחה. ובאם שאשכח לתנאי מודעה זאת, ואדור מהיום עוד, מעתה אני מתחרט עליהם ומתנה עליהם, שיהיו כלן בטלין ומבטלין, לא שרירין ולא קימין, ולא יהון חלין כלל וכלל. בכלן אתחרטנא בהון מעתה ועד עולם.

Listen please, expert judges, every vow or oath or prohibition or Konam-prohibition or ban, that I have vowed or sworn, whether in a waking state or in a dream, or I have sworn by the Holy Names that may not be erased, or by the Ineffable Name of Hashem, Blessed be He; or any type of nazirism that I have taken upon myself, even that of Samson; or any prohibition, even against enjoyment, which I have taken upon myself or upon others, by any expression of prohibition - whether by expression of prohibition, ban, or Konam; or any undertaking, even of a Mitzvah, that I have taken upon myself, whether by expression of vow or of donation, whether by expression of oath or of nazirism, or by any expression, also that which was done by handshake, whether by expression of vow or of donation, or any Mitzvah-practice in which I have conducted myself, and any utterance of my lips from my mouth, or that I have vowed and resolved in my heart to do any Mitzvah of the Mitzvoth, or some good practice or good thing, which I have practiced three times, and I failed to stipulate that it be without [the force of] a vow; whether something I have done regarding myself, or regarding others; both those known to me, and those that I have already forgotten. For all of the above, I have regret for them from the outset. I ask and seek annulment for them from your Honors, because I fear lest I stumble and be caught, G-d forbid, in the sin of vows, oaths, nazirisms, bans, prohibitions, Konams, and consents. Now, I do not regret, G-d forbid, the fulfillment of those good deeds I did! Rather, I regret accepting the matter by expression of vow, oath, nazirism, prohibition, ban, Konam, consent, or undertaking in the heart; and I regret that I did not say, "I am doing this matter without vow, oath, nazirism, ban, prohibition, Konam, and undertaking in the heart." Therefore, I ask annulment for all of these. I regret all the aforementioned, whether the deeds were matters related to money, matters related to the body, or matters related to the soul. Regarding them all, I regret the expression vow, oath, nazirism, prohibition, ban, Konam, and undertaking of the heart. Now, whereas by Halachah, one who has regret for his vow and seeks annulment, must specify his vow; however, please know, my teachers, that it is impossible to specify them because they are numerous. And I do not seek annulment for those vows that should not be annulled. Therefore, please let them seem before you as if I had specified them.

The judges respond by reciting three times:

Everything shall be permitted for you, everything is forgiven for you, everything is released for you. There is here neither vow, oath, nazirism, ban, prohibition, Konam, excommunication, contempt, nor curse. But there is here, forgiveness pardon and atonement. And just as the earthly court permits them, so too may the Heavenly court permit them.

The applicant submits notice for the future:

I hereby submit notice before you, and I cancel from here on out, all vows, oaths, nazirisms, prohibitions, Konams, bans, consents, and undertakings of the heart, that I shall accept upon myself, whether awake or in a dream, except for vows to fast taken at Minchah-time. And if I forget the conditions of this notice, and I vow from today onwards, I regret them retroactive to now, and stipulate about them that they all be null and void, neither ratified nor extant, and that they not take effect at all. I have regret for all of them, from now and forever.
