abaye proves from the mishnah on shabbos 109b that eating shesisa for medicinal purposes on shabbos is allowed if your intention is for eating. what is the difference between this and the lishna acharina?

I suspect that the Gemara only adds the Lishna Achrina (which doesn't seem to add anything to the first Lashon) because it flows better with 'u'Tzericha de'Rav' which follows where the Gemara differentiates between the Din of Shabbos, where it is considered 'Keivan d'Lechatchilah l'Re'u'ah ka'Mechaven' and Rav, where it is considered 'Lechatchilah l'Refu'ah ka'Mechaven'.
