1&2) It seems like they are included in both the case where the sale is on the complete dovecoat and hive, and in the case where the sale is for the benefits (see Tosfos Yom Tov 5:3).
3) I can't imagine that these are included, as they do not fit the parameters of included things, which is "Klei Rechivah" -- "riding tools." The rationale is that as a donkey is for riding, only riding tools are automatically included in the sale (see Aruch Hashulchan C.M. 220:6).
4) The sale of a nursing slave does not include the child (see Aruch Hashulchan C.M. 220:9).
