When referring to lashes elsewhere, the BT usually just says "lashes" but here it says "forty" and "shoulders".
Why mention shoulders? Is this to indicate that the lashes are rabbinic? If so, is there a source that says that?

I didn't find any explanations. It seems Rav Sheshes is simply using his own expression to convey that the person is Pasul.
