Rashi: This is cleaving the innards. The source [to say so] is "u'Keshasos Ne'arim Tartashnah" (Yeshayah 13:18).

Radak: It is hurling [the babies] the babies to the floor or against the wall, cruelly. Yonason translates Tetaref; this is like 'Oh Tarfah b'Kosel' (Chulin 56a).
