(1) The tenth Kidush is done upon removing the gold vestments, and it is not
accompanied by a Tevilah (since he is now going to be donning his normal,
weekday clothing and not Bigdei Kodesh). This chart follows the opinion of
the Rabanan who argue with Rebbi Meir (31b). According to Rebbi Meir,
though, there are two Kidushim done at the time of the *first* Tevilah, and
there is no additional Kidush after the fifth Tevilah.
(2) This is derived from the verse, "u'Va Aharon..." (Vayikra 16:23). Even
though this verse is written right after the Avodos of Yom Kipur (which are
done after the second Tevilah), nevertheless a Halachah l'Moshe mi'Sinai
teaches that the Kaf and Machteh are not removed until after the Kohen's
Ayil and the nation's Ayil have been offered, in order to require an
additional two changes of clothing, and a total of five Tevilos upon
changing clothes.
