(1) It is exempt from a Mezuzah because it is a disgrace for the Mezuzah to be on the door of a place where people go only in a state of undress.
(2) It needs a Mezuzah because by adorning themselves there they make it a "Dirah," unless it is used as a bathhouse, in which case it is exempt from Mezuzah because of the abundance of foul air ("d'Nafish Zuhama"), which makes it disgraceful for the Mezuzah.
