(1) This ruling follows the opinion of the Chachamim, at the start of the Mishnah, who rule "Yesh Ma'amar Achar Ma'amar." According to Raban Gamliel, only one Get is necessary. (RASHI 50b DH Tzerichos)
(2) This is the Halachah as recorded in the Mishnah. However, the Gemara (52b, cited by Rashi here) makes it clear that this is not agreed upon by all. This follows the opinion of Rebbi Akiva, who asserts that Kidushin cannot be effected with Chayavei Lav, and therefore they cannot be effected with a Chalutzah. According to the Chachamim, Ma'amar or Bi'ah that follows a Chalitzah is considered to be a new act of Kidushin, and the Yavam must give her a Get in order to release her (since she is prohibited to him).
(3) This is the Halachah as recorded in the Mishnah. However, from the Gemara (53a) it is clear that this ruling (in the case of two Yevamos and a single Yavam) also follows the opinion of Rebbi Akiva. Even though the second Yevamah ("Tzaras Yevimto") is Asur to the Yavam only with an Isur Aseh (Daf 11a), Rebbi Akiva holds that Kidushin cannot be effected between them (see Insights to 53a). (In the case of one Yevamah and two Yavams, the second Yavam obviously cannot effect Kidushin because of the Isurim of Eshes Ish and Eshes Ach -- see Insights to 50b.)
