One who lost Hashgachah, and is left to chance, even if his affairs succeed based on nature, his spiritual soul senses that his shade (Hashgachah) departed, and it is worried and sad about this.

Why did he say "Yagon bi'Lvavi Yomam"?

Radak: Even during the day, when a person engages in worldly needs and forgets his sorrow, I do not, for I have so many afflictions.

Malbim: My heart is sad even during the day, i.e. when I succeed, for it feels that Hashem abandoned it.

Why does it say "Ad Anah Yarum Oyvi Alai"?

Radak: How long will my enemy rule over me?

Malbim: This corresponds to "Ad Anah Tastir Es Panecha Mimeni." He asks, until when will my enemy be above me; then he will receive punishment via enemies and afflicters.
