(1) This refers to the girl's objection when the brothers designated for her a dowry worth less than a tenth of the estate (i.e. less than her due dowry), or when she was married (Nesu'in) and they did not designate anything for her. (According to the Maharam, the objection of a Bogeres who has not yet married is when the girl says to her brothers *before* she becomes a Bogeres that she wants them to marry her off and give her a tenth of the estate before she becomes a Bogeres.)
(2) This is the Maskanah of the Gemara -- according to Rebbi Shimon ben Elazar, a Na'arah who has married does not receive her dowry even if she objected (as the Rishonim cited by the Shitah Mekubetzes explain). This must beRebbi Shimon ben Elazar's opinion, for otherwise, in what respect is Rebbi Shimon ben Elazar arguing with Rebbi with regard to a Na'arah who has not yet married? Rashi (DH Af), who does not explain this way (as the Shitah Mekubetzes points out), is explaining according to the Havah Amina of the Gemara -- see Insights.)
(3) This refers to when she received a small dowry when she *planned to get married* but did not yet get married -- for if she has already married, she no longer receives Mezonos (see Maharsha). Alternatively, it refers to when the brothers designated for her Mezonos after she got married, even though they are not obligated by the conditions of the Kesuvah to do so (see Maharsha and Maharam).
(4) This refers to when the brothers designated for her Mezonos after she becomes a Bogeres, even though they are not obligated by the conditions of the Kesuvah to do so (Rashi).
