The Ramban says that an mikveh of entirely drawn water is valid. What is the halachah, and how does it apply versus his ruling?

I'm not sure of the source for the statement you attribute to the Rambam.
Water drawn from a well and poured directly into a Mikvah is obviously invalid. However, there is a Halachic concenpt called Hamshachah which, if translated as drawn, could be misleading, for Hamshachah could render a Mikvah made from drawn water valid.
