The Talmud disqualifies an animal for the altar if it killed a person intentionally. What if it backed over a person and killed it unintentionally? Is it permitted or forbidden for the altar. I checked here and on BK 40b and can't find a ruling.

The Rambam Paskens in Hilchos Isurei Mizbe'ach (4:3) that an animal which was spurred-on to fight with people, and the animal therefore became upset as intended and killed a person (presumably not as intended), the animal can be brought on the Mizbe'ach as it is clearly an Ones. The case you are mentioning also sounds like an Ones, which seemingly, according to this Rambam, would make it acceptable as a Korban.
All the best,
